Short version: Bad GM of the "unfair, unfun, and various other "un-s" variety was GMing a Trail of Cthulhu game and made the mistake of pissing off the resident Roleplayer (not roleplayer as in player, Roleplayer as in 'this person's acting at the table'). So in retaliation, Old Man Henderson came into being as a derailing munchkin of a character, which TVTropes describes as an expy of "the Dude from the Big Lebowski, with a lot of the mellow taken out and replaced with pure insanity". His antics included dropping a yacht on a penthouse sweet owned by cultists, smoking a blunt made out of a page of the Necronomicon, turning the campaign from an investigation into open war with the cultists, and actually permanently killing Hastur. A Munchkin Player with an overpowered character of questionable legitimacy is facing off against a Game Master who's treating the game as PvP (or GMvP) and aiming for a quick TPK instead of facilitating a campaign. Basically you've got a GM and a Player who are more or less cheating for the sake of crushing the other side of the table. It's not surprising that they'd come to blows over it.
I thought mages had higher accuracy stats. Then again, we're talking about Celia here, and I could see her rushing the attack rather than focusing.
Also, I have to give props to the DM there for resetting those two clearly overpowered characters.
